How to Obtain Services

Talk to Your Provider

Most referrals for Home Health Services are made by Providers. Referrals can be made easily using our secure fax line at (603)934-1234 or by calling (603)934-3454 and asking for the Intake nurse.

Concerned Family or Friends

Referrals for Home Care or Hospice services may be made by anyone. Families, concerned friends or neighbors can request that an individual be evaluated for assistance by calling (603)934-3454. The agency clinician will talk with the person’s provider to determine whether that person is eligible for Home Care services.
